LIFT is an integrated team made up of Consultant Geriatricians, Occupational Therapists, Physiotherapists, Specialist Nurses and Integrated Therapy assistants working together to ensure the delivery of a high quality, effective service.
We have an exciting opportunity for a new Band 6 Physiotherapy post. The successful candidate will work in LIFT to deliver specialist intervention to the Falls and Fracture Prevention Service and provide comprehensive assessment within the Frailty Team.
You will be based at the Royal Preston Hospital site but provide part of the service at the Chorley District and General Hospital site. The post holder will work in the multi-disciplinary falls and balance clinics which may involve home visits. They will also be required to work with the emergency department and on the Medical Assessment Unit, with the aim of reducing admissions and length of stay.
You should have experience of working with frail elderly patients and have the skill to assess patients with a history of falls. The successful candidate will work within a team to prescribe and deliver effective treatment plans and should have experience of delivering presentations and patient group work.
The organisation, clinical management structure and culture provide a supported work environment where you can develop your skills and knowledge and play a key role in the delivery of high quality health care.
We are moving towards 7 day working, and as such the successful candidate will be expected to support this model of care.
