We are looking for an innovative and enthusiastic Palliative Medicine Consultant to join our team at Lancashire Teaching Hospitals NHS Trust. The post will work alongside another full time consultant post, a clinical fellow post and the CNS team to provide the medical input into palliative care services across the Preston and Chorley hospital sites. We work closely with our hospice and community Services to ensure continuity of care across the health economy and in support of this the Palliative Care Directorate in the hospital hosts all 4 consultant posts across the health economy.
Lancashire Teaching Hospitals is a large tertiary referral centre as well as providing district general hospital services. The Hospital Palliative Care Team is closely integrated with other services in the hospital. We attend daily board rounds and regular ward rounds on the oncology wards as well as Oncology MDT’s and are piloting an enhanced supportive care model within oncology outpatients. We work closely with the non-malignant specialties including ICU, acute medicine, care of the elderly and respiratory medicine with regular meetings to support identification and care for patients approaching the end of life.
We support the development of end of life care trust wide and have a large educational portfolio with a dedicated Education Facilitator. We also have a proactive relationship with the IT clinical strategy and are developing cross boundary IT solutions to information sharing via the Lancashire Patient Record Exchange System.
On call is currently 1 in 5 non-resident providing support to St Catherine’s Hospice and via the telephone advice line to community and hospital services which include 7/7 specialist palliative care CNS services in the hospital and community.
