An exciting opportunity has arisen for a motivated individual to join our established Motor Neurone Disease multidisciplinary team on a 12 month secondment.
This is a very rewarding, privileged role which encompasses every aspect of the patient journey. You will be an integral part of the award winning Care Centre team who strive to ensure a coordinated and multi-professional approach to diagnosis, timely symptom management, interventions, and palliative care provision for all patients diagnosed with or suspected to have Motor Neurone Disease.
You will need to have excellent communication and organizational skills as well as the ability to work flexibly, autonomously and as a core member of the MDT. Experience in acute and rehabilitative neurological conditions is also essential.
Your role will involve assessing people living with MND from diagnosis throughout the disease trajectory. Assisting with provision of a bespoke orthotic management service, especially when patients cannot attend hospital-based orthotic services or live too far so that travelling to the hospital is unviable. Assisting and liaising with wheelchair services when there are complex wheelchair provision issues. Liaison with community therapist, giving advice, support and education to health and social care professionals caring for those living with MND. Assist with the Cognitive triage service.
The post holder must have the ability to work independently both in a hospital and community setting and to liaise with GPs, neurologists and other members of the MDT. The post holder will need to be able to drive to patient’s homes and clinics including Lancashire and South Cumbria.
